Mission
Our mission
at Eveleth Health
Services Park Fitzgerald Nursing Home
is to provide personalized
health services to our residents with a focus on quality of life.
We are a Skilled Nursing Facility (SNF) certified by the Medicare and Medicaid Programs.
We have
a high staff-to-resident ratio which allows us to spend more time caring for
your loved one.
Our staff members provide and promote personalized quality of care in a homelike
environment.
Our residents are provided with daily activities, dining, rehabilitation and
pharmacy services within the facility.
Staff
provides excellent quality of care for our residents and are trained in
caring for their personal, emotional and physical well-being.
Ancillary services include: physical, occupational and speech rehabilitation services.
We
provide Medical equipment , pharmacy and dietetic consultations,
and an on-site Medical Director with lab and x-ray services are available.
Nursing
The
Nursing department includes, the Director of Nurses, Registered Nurses, Licensed
Practical Nurses,
Rehab Nurses and Certified Nursing Assistants to provide care to our residents.
Nursing
services are delivered to our residents 24 hours a day. We have on-call
physicians in the event of an emergency.
Our residents are also seen by their primary physician on-site.
The
nursing staff is provided with on-going education in order to keep our staff
up-to-date in the latest
healthcare trends. We believe that communication between our staff, families,
and medical providers is of
key importance in order to provide the best quality of care for our residents.
Our caring Certified Nursing Assistants assist our residents with their daily cares and provide them with extra TLC.
Nursing staff accompany residents to clinic appointments when needed and on all resident outings planned by activities.
Social
Services
Our
facility provides a Licensed Social Worker to assist in providing for the needs
of the residents
and their family members. Other duties include admissions and discharge
planning for our residents
whether it be back to their own home or to an Assisted Living. Social
Services assures that the individual
needs are met such as: financial concerns, questions regarding insurance and
acting as a resident advocate.
They are involved in the resident’s plan of care, meeting the psychosocial
needs of the resident,
in-servicing staff on issues related to resident
rights and confidentiality.
Activities
The
Activity Department consists of an Activity Director and assistants who provide
and organize activity
and recreation programs based on each resident's interest, strengths and
needs. It is designed to meet the
physical , mental, spiritual and psychosocial well-being of each resident.
A calendar of daily activities is provided
for each resident. Some activities include games, i.e., bingo, cards,
pokeno, board games, thinking games and trivia.
We offer religious services, musical entertainment, special parties and visiting
pets. There are numerous outings
to local casino’s, restaurants, shopping malls, fishing trips and other
local areas of interest.
We welcome and encourage family involvement and participation on outings.
We have
a beauty/barber shop located in the facility where residents may have their hair
set, permed and or cut.
Activity staff and volunteers provide weekly hair care as well.
A licensed beautician is available by appointment for cuts and perms.
Dietary
The
Dietary department consists of a Certified Dietary Manager, Chefs and Dietary
Aides.
We cook homemade meals for the residents, staff and the surrounding
communities.
We average approximately 150 meals per day. Meals are prepared according to
State and Federal Regulations
and follow the Recommended Daily Allowances.
All
menus are written by the dietary manager and reviewed by the consultant
dietitian.
Menus are planned in a five week menu cycle and are changed twice a year to accommodate
the seasons.
Once a
week our management team prepares breakfast for the residents in their dining
room.
The wonderful aromas in the air bring back memories of home.
We
prepare and provide meals for special events such as picnics, B.B.Q.’s., fish
fries,
family events, and the Annual Fall Bazaar held at our facility.

Senior Dining : Area senior diners are invited daily to enjoy a good meal
and
social interaction in our cafeteria on the lower level.
We also deliver meals to the Hilltop Apartments across the street on McKinley
Ave.
Housekeeping
& Laundry
We have full
time housekeeping and laundry staff .

Dialysis Unit
operates 6 days a week - Monday through Saturday.
The
Eveleth Iron Range Clinic
is located next door to our facility.
The clinic is operated by Dr. Edmund Draper.
Claude Erickson P.A. is also on staff. Open Monday - Friday
